Support Services Manager ***$2,000 Sign-on Bonus***

Service Managers are primarily responsible for three areas of Support Services: 1) Providing case management for an assigned caseload of individuals who receive Medicaid Waivered services as designated on each individual’s Service Plan; 2) Facilitating the development and implementation of each client’s Individualized Service Plan through quarterly reviews and regular monitoring; and 3) Developing and supervising a team of Direct Service Providers for high quality service delivery. The Service Manager serves as the REACH primary point of contact for the individual and works collaboratively with family, other natural supports, community partners and care coordinators.

Minimum education/experience:
Two years in a supervisory role or an equivalent combination of relevant work experience, education, and training (at least one year of experience in developmental disabilities) that demonstrates the ability to perform the essential job functions (Bachelor’s degree in human services field preferred).
Physical Requirements: Ability to perform sedentary work. Visual acuity necessary to utilize a computer monitor throughout the course of a normal workday. Ability to perform repetitive keyboarding activities and operate general office equipment. Ability to communicate verbally, both in person and over the telephone. Comfortable providing direct service. May come in contact with blood borne pathogens. Heavy lifting may be required.
Working Conditions: Work is performed at the REACH office and in the field.
