Summary Position Statement: This position is responsible for providing early intervention and physical therapy services to families with infants and toddlers (birth to 3 years of age) who experience developmental delays or disabilities. Services include transdisciplinary play based developmental evaluations, family service coordination, and physical therapy. Evaluations, Individualized Family Service Plans (IFSP), and home visits are completed and implemented with families to promote the family’s capacity to support their child’s growth and development within the child and family’s natural environment.
Essential Functions of the Position:
- Provide services to address the promotion of sensorimotor function through enhancement of musculoskeletal status, neurobehavioral organization, perceptual and motor development, cardiopulmonary status, and effective environmental adaptation. These services include:
- o Screening, evaluation, and assessment of children to identify movement dysfunction;
- o Obtaining, interpreting, and integrating information appropriate to program planning to prevent, alleviate, or compensate for movement dysfunction and related functional problems; and
- o Providing individual and group services or treatment to prevent, alleviate, or compensate for, movement dysfunction and related functional problems.
Other Duties & Responsibilities:
- Participates in public awareness activities and child find efforts.
- Participate in Reflective supervision with the ILP team
- This program uses an infant mental health approach. Support and training will be provided.
- Provides in-service training as necessary within the program, parent groups, community, and at statewide conferences.
- Supervises PT students as needed.
- Understands and complies with all agency policies and procedures.
- Ensures confidentiality as prescribed by agency policy and federal/state law.
- Maintains and promotes a positive professional working relationship with all employees and partner agencies.
- Attends and participates in meetings, committees and training as assigned.
- Performs other duties as assigned by the ILP Director
- Participates in community diagnostic teams as needed.
Licensure: Current Alaska State licensure to practice physical therapy required. Registry with the SEED with an assigned Level 10 or higher within 30 days of hire. Completion of the State of Alaska Part C Credential within 6 months of hire.
Licensure: Current State of Alaska LCSW (Licensed Clinical Social Worker) or license eligible preferred. Will also consider other licensure in mental health such as LPC or LMFT or new graduates. Registry with the SEED with an assigned Level 10 or higher within 30 days of hire. Completion of the State of Alaska Part C Credential within 6 months of hire.
Minimum Education: Master’s degree in physical therapy required. CPR and first aid certificates required within 30 days of hire. TB, fingerprint and background check clearance required. Must have current Alaska driver’s license and auto insurance per agency standards.
Minimum Experience: 1 year of pediatric physical therapy experience required.
Knowledge, Skills & Abilities:
- Knowledge of child development.
- Proficiency in the use of computers for word processing, spreadsheets and other applications.
- Strong organization, documentation, and time management skills.
- Strong interpersonal and written communication skills.
- Detail oriented with the ability to handle multiple tasks simultaneously.
- Effective problem-solving skills.
- Ability to complete required documentation and reports within required timelines.
Physical Requirements: Ability to perform sedentary and active work that requires some lifting. Visual acuity necessary to utilize a computer monitor throughout the course of a normal workday. Ability to perform repetitive keyboarding activities and operate general office equipment.
Working Conditions: Work is performed at the REACH office, client homes, and requires traveling to home visits, evening visits and may include travel to outlying communities by small planes.
Provides consultation to other providers and families. Provides family service coordination for children enrolled in the program, as needed. Develops and implements Individual Family Service Plans. Maintains up-to-date and accurate files and data entry in accordance with ILP and agency policies and follows billing procedures. Participates as a team member in transdisciplinary assessments. Travels to outlying areas as needed to provide services. Develops and maintains collaborative working relationships with other providers and agencies serving families. In doing so, makes referrals and accesses resources for children and families. Work is performed primarily within the client’s home, including evening home visits; and will require driving during inclement weather conditions. Facilitates the transition process for children exiting from the program based on ILP standards. Actively participates in on-going learning, mentoring, and relationship-based supervision.
